PAPERWORK:
A completed set of paperwork and proof of current USSA licence must be turned into the SVSEF office before an athlete will be allowed to participate in any team activity (dryland included!)

REGISTRATION DEADLINE FOR RETURNING ATHLETES: SEPTEMBER 10, 2008

REGISTRATION DEADLINE FOR NEW ATHLETES: OCTOBER 15, 2008

SCHOLARSHIPS: Scholarship Application forms will be available online and in the SVSEF office on August 15, 2008 and are due by September 10, 2008

Travel Estimates sheets will posted below. If you are applying for a travel scholarship, you will need to print the Travel Estimate related to your team and have your coach fill it out.

Scholarships are available for SVSEF athletes for tuition for all teams, and travel expenses for upper teams (B team, J3, Comp teams, and above).  

Scholarships will not be considered unless the following forms are completed (click to download):
            - Tuition and Travel Scholarship Application
            - Required IRS forms (Entire 2007 1040 and all supplementary schedules)
            - IRS 4506

The estimated travel sheet must be completed for travel scholarships. Please download the form below that corresponds to your team and send it in with the scholarship application. Be sure to contact your program director for approval of qualifying events.

SCHOLARSHIP DEADLINE FOR NEW ATHLETES: OCTOBER 15, 2008

TUITION PAYMENT SCHEDULE
All registration materials from returning athletes must be received by SEPTEMBER 10, 2008, or there will be a $60 LATE FEE assessed. 50% of total fee must be paid by October 10, 2008 and the remaining 50% must be paid in full by December 10, 2008. Registration materials for new athletes are due October 15, 2008 with 100% of tuition due by December 10, 2008
(* Fee payment not received or paid in full with regard to the above terms and dates will be assesed a 10% charge on total tuition.)

TUITION REFUND POLICY
There will be no refunds of any fees after December 1, 2008. Written request for refund must be received by the SVSEF office by December 1, 2008. in the event of injury that results in athlete's inability to participate in the program, refund requests will be prorated through Februrary 1, 2009 and all requests must be received in writing.

PAYMENT OPTIONS
SVSEF accepts payment in the form of cash, checks (there will be a $27-30 fee for returned checks), VISA and Mastercard (2.5% fee will be charged), and American Express (3.5% fee will be charged).
